The practice perspective

One review involves more than one task.

A medication review brings together preparation, patient information, pharmacist assessment and work that needs to happen afterwards. Each stage can be complete while another still needs attention.

Hannah Health is being developed to keep those stages connected. Practice staff can see the work being prepared, pharmacists can review the evidence, and the next action retains a clear owner. A completed conversation does not silently become a completed clinical review.

The result is a workspace organised around the work your team needs to do, with the clinical decision kept in the hands of the pharmacist.

A considered workflow

Clarity from preparation to follow-through.

01

Prepare the right work

Preview a cohort before committing it, keep the source reference, and make duplicate, excluded or unsuitable records visible. Technical validation supports preparation; the clinical team still defines suitability and the service pathway.

02

Make outstanding work visible

Keep the difference between information collected, a review awaiting assessment and follow-up still open. Missing or declined answers stay explicit, giving the pharmacist a useful picture of what needs clarification.

03

Keep the handover accountable

A reviewed note, an exported note and a confirmed filing are separate records of progress. Open actions retain their own resolution step, so signing a review does not hide the work that follows it.

Working with your practice

Start with the workflow you have.

A useful discussion starts with how cohorts are prepared, who reviews the evidence and how the practice confirms that a note has reached the clinical record.

Discuss your workflow

What we would explore together

  • The service scope and who owns its clinical decisions.
  • How records are selected and their source is checked.
  • Who handles communication needs and unresolved information.
  • How your team assigns follow-up and confirms filing.

These decisions shape a practical workflow. Hannah does not assume that every practice uses the same process or that an existing clinical system can accept a direct connection.

Explore the current product.

The working product is being evaluated with synthetic records. Patient use requires separate clinical, information-governance and operational approval. The current record handover is a pharmacist-approved note export with manual filing acknowledgement.
