The Hannah Health workspace

Everything in context. Every next step visible.

A considered workspace for the work around a medication review, from the first cohort check to the last open action.

One workspace. A clear next step.

Hannah is in development. This is the actual local application using synthetic records, shown to explain the product’s approach. Read the current status.

Purposeful preparation

Start with the right work.

Useful reviews begin before a conversation. Hannah brings cohort preparation and question selection into a defined workflow, with explicit choices about the work to include.

The local workspace supports import previews, duplicate handling, exclusions and approved protocol revisions. The team can see which questions belong to the selected work and which version was used.

01

Know the scope

Make the purpose and included records visible before collection begins. A list of records is a starting point for review, not a clinical conclusion.

02

Keep questions governed

Prepare and approve question sets, retain their revisions and keep changes traceable. Collection follows an agreed purpose.

03

Keep gaps explicit

An unknown, missing or declined answer remains visible. The workspace does not turn an absence of information into a reassuring answer.

Evidence before decisions

A clear path back to the source.

Patient words, supporting evidence and pharmacist judgement each have a different role. Keeping them connected helps the reviewer understand the record without treating a summary as the original source.

Source

What was said

Answers and evidence stay linked to the context in which they were collected.

Judgement

What it means

The pharmacist reviews the information and records their assessment.

Next action

What happens next

Follow-up has an owner, with open work kept distinct from a filed note.

One record, distinct responsibilities

Signing is a step.
So is following through.

A review, an approved note, a filing acknowledgement and a completed follow-up are different events. Hannah keeps these visible separately so that one completed step does not hide another outstanding responsibility.

The local product includes reviewed-note export and manual filing acknowledgement. It does not claim a live connection to your clinical system. Integration and practice acceptance would need to be agreed before a patient pilot.

A place for the work still open.

  • Review the evidence before signing a decision.
  • Check the note before export or manual filing.
  • Keep the next action, owner and status visible.
  • Close follow-up deliberately, rather than by implication.
